Planning on volunteering? You will need to get your annual background check. See below for details.

Please complete a Free and Reduced Meal Application if you feel that you would qualify for your child to receive free or reduced price meals. The 24-25 application is now available on your Infinite Campus Parent Portal, which is the most efficient way to apply. If you need a paper copy, they are available in all school buildings as well as the Board Office and Service Building.

If you are taking a vacation, a Pre-Arranged Absence Form is required to be completed  for ALL Mentor Public Schools students at least 1 week prior to your vacation in order for the absences to be marked excused. The form is attached and should be submitted to the school.
